The acceptance of new patients varies upon the availability of openings specific to each individual physician.   We welcome you to complete an information sheet in order that you might be put on our waiting list.  Please mail to the address at the bottom of the form or fax to (765) 741-4658.  A member of our staff will notify you as openings become available.

Sometimes there just aren’t enough hours in the day to get it all done.  No worries, our portal is available 24 hours a day, seven days a week.  If you need a refill, an appointment, or to just send a note to your physician you are able to do so at your leisure.  We will get back with you the next business day.

Thank you for choosing Woodway Internal Medicine.  We are very appreciative that you have chosen our facility as your new health care provider. 

To better assist our patients we have compiled some basic information to facilitate a smooth process on your first visit.  You will be mailed a new patient packet to read and complete prior to your initial visit with our practice.  In the meantime, please follow the instructions below to make your visit as easy as possible.

  •  Arrive 15-20 minutes early for registration

  •  Bring ALL your medications with you

  •  Bring all Health Insurance cards

  •  Bring a picture ID

  •  Bring the completed patient information sheet

  •  Bring the completed patient intake form

 **Woodway does NOT accept Caresource Insurance or Medicaid.  Anyone choosing these

plans  will need to transfer their care to a Caresource/Medicaid network provider.  

No exceptions.**

What if your plan isn’t on our list?  Woodway will submit a claim to any patient’s insurance company with the exception of Caresource and Medicaid. Even if we are not a participating provider, plans will often cover a significant percentage of our bill.  However, payment by your plan may be at a lesser percentage and require that an out of network annual deductible be paid by the patient.

We recommend that all patients check with their benefits representative at their health plan prior to seeking Woodway care.  If your insurance plan requires network participation, please confirm that our providers are on the list.  **Please be sure to confirm that the physician is listed with our 4000 W Woodway Drive address.  Several of our physicians are also contracted through Ball Memorial Hospital, and those affiliations do not apply to their private practice billing.  We will always do our best to maximize the health plan benefits you have.

Note Woodway may not be taking new patients for all contracted insurances.
